Reasonable Accommodation 10/27/2008

If you require a modification to fully access and utilize the services at St. Francis Center (SFC), you may request a reasonable accommodation, which can include:

  • An exception to a rule, policy, or procedure
  • A change in the way we communicate with you or give you information
  • A change or repair at our facilities

To qualify, you must:

  1. Have a disability under the following ADA definition:
    • A physical or mental impairment that substantially limits one or more of the major life activities; or
    • A record of such impairment; or
    • Regarded as having such an impairment

  2. Submit a Request for Reasonable Accommodation form (staff will help reduce to writing if necessary)

  3. Have a qualified physician or other professional verify that you require the accommodation due to your disability and the change is required for you to have equal access to SFC services.

If you qualify, and it would not create an undue administrative or financial burden, SFC will make every effort to grant the request or present an alternate accommodation that will still meet the needs of the request. Once you submit a request, the Section 504 Coordinator will confirm the receipt of the request within one week and will notify you of the decision within 30 days if no additional information is needed and no verification is outstanding. The notification will state whether the request has been approved or denied. If denied, an explanation will be included as well as information about how to request an informal hearing to have the decision reviewed.
